What Is Scalp Micropigmentation?

Scalp micropigmentation (SMP) is a cosmetic tattoo procedure that deposits tiny dots of pigment into the scalp to replicate the appearance of hair follicles. When done well, it creates the look of a closely shaved head or adds the appearance of density to thinning areas. It's not a hair transplant - no hair actually grows. But the visual effect can be remarkably convincing.

How It Works

A trained SMP practitioner uses a specialized tattoo machine with fine needles to place pigment dots in the upper dermis of the scalp. Each dot is sized and colored to match the look of a real follicular unit. The procedure typically requires 2-3 sessions spaced 1-2 weeks apart, with each session lasting 2-4 hours. The first session establishes the foundation, and subsequent sessions build density and refine the hairline.

Who Is It For?

SMP works for a wide range of situations: men with advanced hair loss (Norwood 5-7) who lack sufficient donor hair for transplants, people who want to camouflage hair transplant scars (both FUE and FUT), women with diffuse thinning who want the appearance of fuller density, and anyone with alopecia areata patches. It's also popular as a complement to a hair transplant, adding the appearance of density between transplanted hairs.

Results and Durability

Results are immediate - you walk out of the first session with visible improvement. Over 2-3 sessions, the result is refined. SMP pigments fade gradually over 3-6 years, depending on sun exposure, skin type, and the inks used. Touch-up sessions every few years maintain the look. Fading is actually an advantage - it means the look stays natural as your face and remaining hair change over time.

Cost

SMP typically costs $2,000 to $5,000 for a full head treatment in North America. Small areas (scar camouflage, hairline only) may cost $1,000 to $2,500. This is significantly less than a hair transplant and requires no downtime. Touch-up sessions every 3-5 years run $500 to $1,500.

Limitations

SMP looks best when the surrounding hair is kept very short (buzzer length or shaved). If you keep your hair longer, the contrast between real hair and dots can be noticeable. Color matching is critical - the pigment should match your natural hair color and skin tone. Poor color matching or heavy-handed application looks artificial. As with any cosmetic procedure, practitioner skill is everything.

Finding a Good Practitioner

Look for SMP artists with extensive portfolios showing healed results (not just fresh work, which always looks better). Ask how many procedures they've done. Check reviews from multiple sources. Avoid practitioners who work from a standard template - every scalp and hairline is different. A good SMP artist will customize the treatment to your head shape, skin tone, and remaining hair pattern.
